What can be a confirmation depends on the text of the review. Usually, it is enough to confirm the fact of admission with a document for verification. In some cases, in order to better understand the problem, you will need to show a statement with a diagnosis, an appointment, a certificate of testing, etc. It is very important that the document shows data such as the date of admission, the doctor's full name (if there is a review of the doctor), the seal of the doctor or clinic (or the signature, if we are talking about research (ultrasound, ECG, etc.)).
How to confirm the fact of admission 
What can you have left after your doctor's appointment? A lot depends on whether you were in a private organization or a public one, as well as on how you received assistance: under compulsory medical insurance, VMI, or for a fee. Polyclinic under the compulsory medical insurance policy

The patient's relatives can receive all these documents upon application to the hospital.
You can request an extract from your medical record or a certified copy of it. To do this, you need to write a statement (there are many templates on the Web) and wait. The time required to provide this documentation may not exceed 30 days.
If they refuse to provide you with documents after admission or give you unverified documents, this is a reason to be wary. Only with the help of a document can it be proved that the interaction with the doctor and the clinic took place.
Electronic documents 
Government clinics: if documentation is kept in electronic form in your region and patients can access an electronic medical record (EHR), then screenshots from it can serve as confirmation of admission: both the conclusion of the appointment itself and a screenshot with information that it took place. What should be in the frame: the address bar (so that it is clear from which site the screenshot / photo was taken), the date of the appointment, the doctor's full name, and an indication that the appointment took place.
Screenshots from the website or from the application can also be taken as confirmation«Госуслуги». In addition to the conclusions, patients can also receive a notification about the opening of a sick leave.
Private clinics: if the clinic has completely switched to electronic document management, then the following can serve as confirmation: a screenshot of the doctor's report (signed with an electronic signature), a screenshot of the doctor's report (without an electronic signature) + receipt of payment for the reception, video screenshot from the app.
